Supplementary Information Form for Students with Disabilities (SIF-D)

The University of Guelph-Humber strives to provide equal opportunity for academically qualified students with disabilities. You are encouraged to complete the Supplementary Information Form for Students with Disabilities if you believe that your admission average does not reflect your true academic ability as a result of a disability.

If you have an admission average of 70-74.9%, or if your admission average does not meet the cut-off for the program to which you have applied, it is strongly recommended that you submit a SIF-D. Your SIF-D may be considered in March if your admission average is within a discretionary range of the determined cut-off (up to 5%). The form is available online (in November) and considered along with your marks in determining admission. The deadline to apply is March 1, 2024. Optional to complete.
